ACADEMIC CONTRACT
 

FOR STUDENT TO CONTINUE IN ATHLETIC & EXTRA-CURRICULAR ACTIVITIES
 
 
 
Through the signature(s) below, the individuals agree to the following terms for the probationary period for allowing the student to participate in a co-curricular activity, including interscholastic athletics.
 
The Student:
 
1.   Recognizes that achieving a passing grade in all of his/her subjects is a pre-requisite to participation in an extra-curricular or interscholastic athletic activity
2.   Will attend daily sports study hall or work with the appropriate tutor during said probationary period
3.   Will make satisfactory progress towards achieving a minimum sixty-five per cent (65%) average in all of his/her courses; satisfactory progress will be determined by the teacher and the building principal
4.   Will obey all co-curricular rules and regulations as established by school district policy
5.   Will prepare a plan for academic success with an administrator
 
If the student fails to complete this contract, he/she becomes ineligible for participation in any co-curricular or athletic activity until he/she has a minimum sixty-five per cent (65%) average in all of his/her courses at the conclusion of a ten-week marking period.
